2021 Design Sprints: Building Cross Sector Partnerships to Address SDOH

Supported by a grant from the Robert Wood Johnson Foundation, NACHC and AAPCHO created a series of Design Sprints in 2021 to help community health centers and their social service sector partners better align to address social determinants of health and advance health equity in their communities.

Health centers and their partners participated in cohorts focused on different pillars of RWJF’s Theory of Change for building effective cross-sector alignment: shared purpose, data, governance, or financing. These podcasts feature health centers and their community partners sharing their experiences addressing SDOH in their communities, strengthening their partnerships, and participating in the Design Sprint program.
